This one page document is now for sale on eBay.  I was wondering, does anybody know specifically which telephone manufacturer distributed this card with their sets?  In other words, exactly who printed this?

It looks like the font used by BECO (Bohnsack Equipment Co.) They were located in Germantown, NY. They refurbished phones and manufactured butt sets.
