Honestly? I am still working it out. There are some limited instructions in BSP 503-100-100 which can be found in the TCI library.

Basically, there are guides that fit the housings of various phones on the top - you put the phone housing into the appropriate guides, reach the tool into the appropriate marked hole as far as the guide will allow, and supposedly it will cut the hole in the correct place.

I remember when I first started with Pacific Telephone a few techs notching old cases.  The guy training me showed me the tool and said there was a template,  but none of the installers used them  because the cases would often crack.  He showed me how to do it with a pair of diags. Luckily, I did not have to do it because new modular sets were always available.
