This is a really cool dial card on an eBay listing. Does anyone have any info?

I'm not an expert, but based on my observations, isn't it a 1958 phone, and maybe refurbished in 1973 not 1978?

It looks like it was pieced together by a refurbisher:

Base - Kellogg
Handset (plastic) - Northern Telecom
Housing - Stromberg Carlson (?)

Area Code 602 is the Phoenix, AZ area.

Access Code could have been for a dial-around-code (prefix) to specify subscriber's long distance carrier, such as MCI, etc.
