How was the 41A mounted to the wall, I'm looking at one that only has 4 holes in the bottom, nothing on what I assume is the wall side or back. Were they, mounted by a bottom bracket, or  what ?

This is a "G-mount" hanging handset, or "211."  The "41A" designation is the number for the moveable dial mount.  They were mounted to an L-shaped bracket, just as you guessed.  The bracket gets mounted to the wall or desk, then the phone gets screwed to the bracket using all four holes on the bottom.

Here's the bracket you're looking for, but it shouldn't be that big a deal to fabricate one.
