It is put out by AT&T. SPIRIT Alert Module. it has a sticker that says 112B interface Unit.Is this for a PBX?

I'm going to guess that's an analog terminal adaptor.  You put it on a station port on the Sprit system, and it enables using an analog phone and ringing an analog ringer.  That's a guess only.
