Looking for 622-100 Subdivision of GTE Practices

Dear Forum:

i am looking for the GTE Practices subdivision 622-100 covering the repair and return of customer owned equipment mentioned in the GTE Practice covering Deco-Tel Specialty telephones, please.

I am wanting to add these to my GTE Deco-Tel practices as additional reference.

if any forum member has these, or can "point" me in the right direction, please I would be most thankful.
