Never seen one like that.  Apologies for making you look when I don't have any helpful information to offer but maybe a bump will prompt some informative responses.

Dan, I also have never seen one of those.  I have worked a few times with a G6 handset---for hearing impaired but in those instances the transmitter cups were modular.    Paul Fassbender has a few of those handsets listed on his site:

but I don't see any photos of the cups.  I'm sure that if you e-mailed him this photo he could shed some light on where/which handset it belongs.
