I just recieved a #6U dial in the mail.  It is marked 12-74 with a 164A dial plate dated III 1973.  It has a faux brass fingrwheel made of either plastic or plated white metal and a brass card holder.  What is it from?

I have a design line "Candlestick" phone in red, white and blue from 1973 that uses that dial.  I think that's where they were mainly used.
