I bought this (ebay link) phone last week for about $32 including shipping.  I cannot find any model numbers, but based on what I found online, it appears to be a GPO no. 366.  The phone is in good condition with some scratches and scrapes, and it does work.  I replaced the handset grip with an NOS Siemens Brothers grip which I bought from a forum member a couple of years ago for another GPO phone.  The handset cord which came with this phone was the Western Electric type and badly stretched, so I replaced it with a plastic cloth strange cord which I had in my spares.  The phone was quite dirty, so I gave it a thorough cleaning and polishing.  I used some English Greygate bakelite polish as the final polishing step.

I am guessing that this phone was exported for use in Canada because it has a North American dial plate.  I need to find an appropriate dial center card.
