Strange looking AE type 38 handset

I bought a metal wall phone that I'm just now getting around to.  It has the stangest looking transmitter.  Has anyone seen one like this?  What would it have been used for?

Does it appear that there is anything besides the bakelite trans cap that is different on these?

the transmitter element is different.  You can see it has a small hole that matches the small hole in the bakelite mouthpiece.  The diaphram seems to be underneath that brass plate with the small hole.  The receiver seems to be like my other type 38 handsets.
