Try these:

http://www.btcnetworkservices.com/dialcard/dialcard.html

http://www.btcnetworkservices.com/dialcard/dialcard1.html

The images that site generates are only 96 PPI. 300 PPI is generally considered the minimum for good print quality.

Earlier today I made a vector graphic of a dial card like the one in your first link. Vector files will max out any printer's quality capabilities by default. If anyone wants to use it, I've attached it here as a PDF (which can be opened in any PDF software, or in Adobe Illustrator if you want to add numbers/text to it), as well as an SVG (can be opened in Inkscape, which is freeware), and as a 1,200 PPI PNG (can be opened in Adobe Photoshop or any other raster graphics software).

I think the easiest program to use to make your own dial cards is Microsoft paint, I think they come out looking very authentic.
