FOUND *** Wanted: WE444 "Designation Card" or Photo of

I'm looking for a WE444EC  "designation card", that's the card under the clear plastic which has the word "HOLD" and then 3 dividing lines separating the buttons like the one below. I don't really expect someone to have one to sell or give away but, what I would like is for someone that has a pretty clean one to take a good photo of it without the plastic cover on it so I can clean it up in Photoshop and try and print a few on card stock. Thank you for any help!

It is an E-2804 Designation Strip. The illustration in BSP C37.321 is not very helpful.

Yes sir, I seem the proper form number for it in the BSP I have but, didn't figure seeing you can't order them anymore that it do any good for my purposes. Thank you just the same and your correct the PDF of the form is petty bad sadly, need a real one or a photo of one.
