I've seen a few old movies in which little old ladies stood on little footstools to reach the transmitters on similar phones. 

There was an "official" height, actually.  It was 56.5 inches--not 56 or 57, but 56.5 :) --to the center of the transmitter.  But a note is added that if this height is not satisfactory to the subscriber, then it should be placed to suit.
