ALright, it is a 151 conversion, at least according to the perch stamp.  I have no idea what the "Y' means.  I will have to add this one to the perch markings section!

Well, at least 2 wires are connected right.

The wiring has me stumped. I don't even know where to begin...

Still waiting for a photo of the switch contacts. But if it's a 51AL:

The green receiver wire on GN and the red harness wire on R terminal block are correct.

1. Move white receiver wire from BK to W
2. Move black harness wire from BB to BK
3. Move brown-blue harness wire from Y to BB
4. Move jumper from GN to BB on dial
5. Connect cord from subset: Green to GN, Red to R on terminal block, Yellow to Y.

Thanks, guys! I couldn't sleep last night so I got up and started documenting what I knew and what I didn't understand.

I found a good line drawing of a 4H dial and began tracing wires. I decided that White to BK was wrong. This meant a cascade of wires needed to be moved. I was sure glad to see these additional instructions this morning!

The diagram shows a 2A dial. Since you have a 4H, you will need to strap BB and R dial terminals. Since there is already a jumper on R, you just to move the other end of it from GN to BB.
