It has a Bell System Insignia in the middle, I guess not

AHHH, OK. There was no pictures when I first saw it. Now I see.  Very Cool. Is there any dates anywhere? I think it is an early one. I think the later ones did not have the ribbing. IIRC they used to come that way when new, you had to punch out the center.

These were used to cover unused jack location on prewired homes. If you added an extension to the room later, they would swap them out with a plate w/center hole or a plate with a 4 prong jack. They also came in blank and with a center knock-out for a 4 prong jack.

Most pre wired  houses I have seen were wired as a big loop with quad cable.
I have seen some that used unjacketed multi-pair looped thru the house. I think it was 4 or 6 pair.
The modern way was to home-run each cable to the Dmarc.
