I didn't want to leave this phone lying around in pieces any longer, so I did some quick sanding and polishing and reassembled the phone last night.  The piece that attaches to the shell below the cradle had fallen off a desk into a pile of stuff, and I spent quite a bit of time trying to find it.  The phone is all back together and working perfectly now.  There are still a few small dents and scratches, but the phone no longer looks like it was polished with steel wool.  I expropriated a four-conductor red line cord from a 500 and used a spare "Please Answer Promptly" dial center.  I briefly connected a transformer to the black and yellow wires, and the light worked fine.  The bulb is marked "GE", so I wonder if it is a replacement.

GE is probably the original supplier for the lamp.

8.01  For the 500H illuminated dial set, a 6- to 8-volt ac or dc
         power supply must be provided to light the dial lamp
(GE #46 MAZDA). The dial lamp lights when the handset  is
removed from switchhook.
