It is stamped on the card, Under the bell, to the right of the clapper. It appears to be manually stamped ( a little off-slant).

That was auction contest #168 ending 3-26-15, just under a year ago, and it did sell for a mere $807.22.

Yes, probably.
This is the typical card that was inserted in display sets that AT&T gave to museums for example.
I doubt the card was installed at the factory though.  The sets that I have observed that appeared to be factory fresh with box, had the card holder frame contents in a separate bag.  Number cards were still in the stock strip of several (four?) number cards, that were perforated for easy removal.
