Northern Electric Call Director face plate card

Hi Al,

I am restoring a Northern Electric call director phone, and need the card that fits under the clear faceplate. Does anyone have a true size scan that can be e-mailed to me of this card, so that I can have one printed on card stock for my phone? Also what color card stock should I use?
