Here is a link to a most often used modern adapter seller but this may not be what you are looking for.

They are $16 for 5 with free shipping, reasonable for a small quantity - $3.20 each.

I think I will go with the 4-prong plug (for historical reasons) and the RJ-11 pig tail adapter.

They used to make a cover for the connecting block that converted it to a 4 prong jack.  I would try to find one of them on E-Bay, and, if your connecting block is not the 42A type used by the phone companies, get one of those as well.
