It is a mouthpiece and adapter. They sold adapters for various brands of tranmitters and they sold the  replacement mouthpieces.

I do think it's a little over-the-top for a mouthpiece originally used for sanitary purposes.

All things considered they may of paid a bit high, but they didn't overpay.   If you have a "need" for this item, It was cheap. My guess is the buyer needed it. I think the buyer is very happy.
