My 202 went pop... need some wisdom

Got this nice 202...
built a sub box and hooked it to my 616.
At first, it worked fine. Then...
It stopped working.
Wondering what is wrong???
Used the following wiring

To what did you have it connected?
Pabx or what?

make sure your caps are rated for 250V or greater... thats a start....

I'll second that as the quoted voltages for capacitors are DC and that poor 2.2uF will have been running at over its max rating.

Used the following wiring

Rod....Looks like you have the Green from your handset cord on the second switch that is not used at all when you have a dial. It should be on Black on the dial.

Where did you get those cords? ..Doug

Doug I think that is a 4 wire cord in a 3 wire connection.
it is just there as a holder.
