Here's a payphone operated by Bell Canada that was removed sometime between May, 2012 and July, 2014. This phone was located at/near 2500 Eglinton Ave. E in Scarborough, Ontario, and was featured in Strange Brew (the movie starring Bob & Doug McKenzie from SCTV - screenshots attached). At some point, the enclosure was changed from an ADCO 120 Diplomat to the pedestal type shown in photo #4 and the phone sign removed.

Ohio:

- Blue Room Lounge (I can't find a picture of this phone) - Main St. & W. 2nd Av., Cedar Falls - Probably Working (It is/was operated by Qwest/CenturyLink/Lumen and still has a Bell enclosure)

Illinois:

Here's 2 out of service Northern Telecom Millenium payphones at the Aulani Disney Resort (92-1185 Alanui Dr., Kapolei, Hawaii, 96707). The phone on the far left (out of frame in photo 1, visible in photo 4) had the number 808/671-1112 and the one on the far right (the one with no handset and GTE/Verizon/Hawaiian Telecom branding) had the number 808/671-1113. The phone in photo 3 is one of the 2 courtesy phones in the bank of phones. These phones seem to be the ones that are more commonly found in Canada that have the keyboard instead of shortcut buttons.

Here are some phones I found on my trip to Columbus, OH:

Photos 1-4: Central Ohio Fire Museum (No Dial Tone) - 260 N 4th St., Columbus - This phone is located in the back of the museum across from the display of melted household items near the door. It also still has the Ohio Bell, Ameritech, SBC, and Sprint branding.

Enclosures/Backboards Only:

Amoco Station (Google StreetView: http://tinyurl.com/yc3cduue)- 4755 Refugee Rd., Columbus - This phone backboard is located on the leaning pole at the rear of the station. This Amoco used to have another payphone, however any remnant of it hasn't existed since 2012.

1020 E Broad St., Columbus (Google StreetView: http://tinyurl.com/2kmpd3er) - This phone stand is located next to the street lamp at the street corner.
