WANTED *** Western Electric Touch-A-Matic Memory Index Sheet

Dear CRPF:

I'm looking for the index sheet that you'd write numbers onto before storing them in a Touch A Matic 32 adjunct dialer or TAM-32 Rotary or tone memory phone.

The card was made from a plastic (mylar) I think that could be written on or typed on, and erased as needed.

It also had labels for specific buttons with specific uses. There were holes for the dialer buttons as well.

I am looking for 1, hopefully 2 so I can have one to put on my TAM-32, and one to make copies of, if I need them.
